In addition to the services provided by Care Partners, Upperline Plus connects your care outside of those appointments to help you stay healthy, and coordinates between your providers so nothing falls through the cracks.

By signing a Voluntary Alignment Form and enrolling in Upperline Plus, you become eligible for the following healthcare benefits that make it easier to stay on top of your health: unlimited telehealth visits, 24/7 access to a provider, pharmacist consultations, home visits, nutritional assessments and dietician services, one-on-one texting and email with healthcare providers, and more. If you have Medicare or select MA plans, there is no change to your existing healthcare coverage, no change to your doctors, and no restrictive networks.
