Who is eligible for Upperline Plus?

Upperline Plus is available to many patients receiving care through Upperline Health and Care Partner provider locations. Talk to your Upperline Health or Care Partner provider at your next visit to see if you’re eligible.

You can continue to see your current doctors, including your primary care provider, while gaining access to additional, wrap-around support through Upperline Plus.
